By completing this form and signing your child up, you give consent for the named child to attend JONES JR Sports Ltd sessions and understand that JONES JR Sports Ltd are under no liability whatsoever in respect to any personal loss or injury which the above named participants may sustain during the sessions. You also hereby authorise the coaches to act for your child in their best judgement in any emergency situation requiring medical attention.
